Visit your nearest branch to apply and apply in person! See branch locations.
Call
Too busy to visit a branch? Call us at 623.580.6000 or 800.224.3330. A Member Service Representative will send you everything you need to apply for membership.
Mail
Or, print out this Membership Application online, fill it out completely and mail it to us with a copy of your driver's license* and a $25 check. This $25 will be deposited into your savings account and establishes your membership with us.

*Important Information About Procedures for Opening a New Account! What this means for you: When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.
Member Eligibility
The following groups are eligible for Canyon State membership:
1. Most Arizona residents (if you live, work or worship in almost any area in Arizona, you're eligible) Please Click Here for eligible zipcodes.
2. Any active, retired or contract employee of:
  • The State of Arizona or governmental units thereof, except employees of state colleges and universities.
  • Safeway, Inc. (Phoenix Division)
  • Other Select Employee Groups
3. Any individual who is a member of the immediate family or household. (Immediate family includes spouse; natural or adopted children; foster or stepchildren; parents and parents-in-law; grandparents and grandchildren; brothers and sisters; stepbrothers, stepsisters and stepparents; brothers-in-law and sisters-in-law; and sons-in-law and daughters-in-law.)
4. Any individual who is the surviving spouse of a deceased member.
Once A Member, Always A Member
Canyon State has adopted the concept "Once a member, always a member". If you leave your sponsor group, you can still enjoy all the benefits of Canyon State membership and are encouraged to use all our services. You do not have to close your account.
