Reporting Identity Theft or Fraud

Step 1: If your Canyon State Credit Union credit cards are lost or stolen, report the loss immediately by calling 800.442.4757 for Visa Platinum Credit Card or MasterCard.

Step 2: If your Canyon State Credit Union debit card is compromised, lost or stolen, report the incident immediately by calling 800.224.3330 or 623.580.6000.

Step 3: Contact all three national reporting agencies to report the theft of your identity. You might start with a phone call to each and follow up with a letter.

Step 4: Alert your financial institution(s). They will close your account(s), open new ones and order new checks for you if necessary.

Step 5: File a police report at your local police department. Keep a copy for your records.

Step 6: File a claim with the FTC at 877.ID.THEFT or www.ftc.gov/idtheft.

Step 7: Complete an ID Theft Affidavit to provide to each creditor.

Step 8: Know Your Rights

  • Under federal law, you are not liable for more than $50 of unauthorized charges to your credit card.
  • By law, once you report the loss or theft of your card, you are no longer responsible for unauthorized charges.
  • If you have unauthorized charges on your card, do not pay them. Call your credit card company to discuss the problem then follow up with a letter. Send the letter via certified mail with return receipt requested. Keep a copy of the letter with the receipt.
